-For instance after Quiet garrottes the doctor, she says over the radio "not yet - the patient in the next bed saw my face" - then she points her gun at YOU and not Ishmael! This implies that Quiet's target was really Ishmael and that she knew he was the real big boss and that Ahab was a fake. After all, if she was sent to kill Ishmael it wouldn't matter what order she shot you guys in.
Plus when Ahab and Ishmael are hiding among the dead bodies, the XOF soldiers shoot all of the corpses to make sure they're dead, but STOP once they reach you - even when they flip over Ahab and he reacts by shielding his eyes against the soldier's flashlights THEY DONT SHOOT HIM! I'm guessing Skull Face wanted Snake alive so he could monologue to him before killing him himself. Or maybe he never wanted Snake dead and just wanted to make his life a living hell?
in 1999 my country was bombed and you cannot compare reasoning of people in that times and your critic mind behind the table. As there are innocent people screaming all the way you cannot really be calm and keep your mind straight to do everything perfect, soldiers are trained for combat, every single war veteran describes killing innocent as probably the worst thing, they always remember that and not combats and such. It effects the mind tremendously
point of scenes you described is that Big Boss got out of there by luck, it was really close but he made it. By making it real close call it should effect player's tension, you are pretty much cornered. How the hell you expect to make a dramatic scene with him not even able to walk properly? All that is used to make you more tense, you are so close and if something happens you cannot really do anything about it, desperate last call for run.
Hallucinations are intro into some other part of the game, and big whale is just another reference to Moby D1ck
That's also why Mantis betrays Skull Foce for Eli at the end of Chapter 1- because Eli is ALWAYS pissed off.

ESPECIALLY when another patient grabs your leg underneath the bed and asks you to help him clearly loud enough for the other guy to hear you
THEN the soldier walks over and drags him away from you while HOLDING your leg and then shoots him twice
From that soldiers point of view you would have been COMPLETELY exposed underneath the bed and THEN he was also holding your leg!
The soldiers were talking in the hallway and were able to hear each other also so hearing wasn't a problem for them
