Here's how I fixed:
1. Go to (C:) > Users > Your User Name > AppData > Local > 4A Games > Metro 2033
2. Open user.cfg with Notepad and change these :
r_fullscreen off
r_res_hor 3840
r_res_vert 2160
Save and in Properties check "Read Only"
This will put the game in Borderless windowed and the correct resolution.
3. To Go back full screen if you need it, download and use the software "Borderless Gaming" (You need to add Metro 2033 to the list inside the software)
4. This method worked for me to play in 1440p but problem: Can't use VSync anymore, even with User.cfg, Nvidia Inspector or Nvidia Control Panel. So to solve this you can do this :
Use Msi Afterburner, Go in Settings, On Screen Display, In the Bottom click "More" there you can set Framerate Limit to 60 and Scanline Sync to 60 and use a Key to toggle on and off.
All of this helped me play the game at 1440p / 60 locked without issue on my 4K Tv.
4A Games, please fix this.
Unfortunately, these methods did not allow me to change the resolution to 3840 x 2160.
I'll try other things, but I hope it's fixed soon.
disable all scaling in the nvidia control panel:
size and position - no scaling, set display to do scaling, uncheck override game and app scaling
sharpening option - disable, no scaling
disable dsr and dldsr if enabled
if still not working, then:
restart the computer
under the windows compatibility options set to let the game exe control scaling
disable steam cloud sync
delete all custom resolutions in the nvidia control panel
reinstall nvidia drivers
turn off your secondary display if you have one
I loaded up CRU for custom resolutions. The top box has 'detailed resolutions'. I had one for 3840x2160 and one for 1440p too. I never set those up as far as i'm aware. They must have been default. I deleted both of those and ran the CRU program for restarting your display driver and now the game recognises 3840x2160.
All the previous custom resolutions i'd set in the bottom CRU box 'extension blocks' I've left, so they didn't appear to be the issue.

Anyone find a fix for this? I'm using an LG CX TV. Doesn't matter what scaling options I use in Nvidia control panel, what config file changes I make and make the file read only, setting custom resolutions from other guides. Absolutely nothing lets me run this at a resolution above 1600x900.
Can't get and other resolutions to show up in options and changing it in the config file seems to have no effect on the game.
