It's weird, the game has perfect ultrawide support, the effects cover the whole screen (i.e. when the mask visor gets messed up), menus are properly scaled and IIRC, even correctly positioned. And still, this issue exists, and it was pretty hard to get around it. Last Light does the exact same thing.
And no, I don't have a second monitor, just a single 21:9 screen.

A LOT
Go to C:\Users\xxxx\AppData\Local\4A Games\Metro 2033\ and edit the file user.cfg, set "r_fullscreen off"
If you're lucky it will already work, but on my system the image didn't fill the whole screen. If so download "borderless gaming" https://github.com/Codeusa/Borderless-Gaming/releases and voila, it's fixed.
Yup, this worked for me; while modifying that file, I also adjusted the fov to 54, which is a bit better for 21:9. On my machine, it did not revert to a windowed mode, which is odd - it's still fullscreen, but now it is the correct aspect ratio.
