- Press Q until it says 'Realistic Gearbox' mode (This will only affect the current session).
- Go to Options > Gameplay and set your default gearbox mode (That will affect all future sessions)
Then you should be using a vehicle with a manual gearbox.
If you use a vehicle with an automatic gearbox, you will see automatic gears (P R N D etc)
Can you please describe what keys you are pressing exactly? Maybe record a video?
Here's a video showing how the realistic (manual) gearbox mode works normally:

All I'm doing in just pressing the clutch and tapping X or Z to shift up/down.

If I am cruising at 1,800 RPM in sixth gear, and I want to accelerate, I have to shift to a lower gear. Running through all the gears to second is inelegant and slow, so I would much prefer to be able to choose whatever gear I want at any time. The strange semi-automatic setting everyone recommends is not ideal.
The game seems to support a true manual style selection of gears, but in order to use them, I have to hold the button for each gear down.
This is very weird to me, so my question is: Can I set the transmission so that when I select the gear I want to be in, I don't have to hold down the button to stay in gear?
You want for example to use 1,2,3.. on your keyboard to engage the 1st, 2nd, 3rd.. gears.
Sorry, I did not understand that earlier.
That's by design

These bindings are meant for H-Shifters not for keyboard usage, so as soon you release, it disengage the gear.
It's not possible to do what you want with a keyboard.
I don't suppose it's possible that at some point in development what I want will be added?
Depends on how many request we receive about it and etc..
Not all keyboards have a minus near the backspace (mine doesn't), so that could rather be a problem with a specific layout.
Here's a picture. Sorry for bad camera and worse mouse handwriting.

Go to Controls menu > Hardware and press that key.
What key does it detect? And if you press the other minus?
