One thing that concerns me is in that same room where you find the wrench (behind the broken, electrified cables area), there's a red glowy area on one of the boxes in that room, but there's nothing there to actually interact with. Same red glow I've seen on some notes in other areas, but in this case there is no note... or anything at all. I've tried both checking it while low on sanity and also while... mostly sane (?) to see if that had any effect, but in either case there's just hasn't been anything to interact with there. Starting to think this may be a bug at this point...
In my case, it was in the bathroom, near the entrance. It was REALLY easy to miss, since the socket blended with the background

EDIT: as Red Panda stated, for me, on the second time, it was in one of these storage rooms...
As some feedback, I'm enjoying second time through WAY less because of the randomisation - There are a couple of key cards for rooms I haven't been able to find this time through, I have FAR less ammo the second time through, and I found it impossible to the second half of the paper for the elevator key card... I don't understand why this would be designed this way when most everything else in this game was so enjoyable the first time through
