Armor keeps unequipping

I bought the game a couple of hours ago. I am a knight and have some plate and rogue's armor pieces. I equip the stuff I want, but every once in a while when I check my inventory I find that some piece of armor was unequipped.

Why does this happen?
Is this a bug or game mechanics?

It's a bug. If I close the inventory while the mouse is over an equipped item, it will automatically un-equip next time I open the inventory.

And it happens when I map Tab key to open inventory instead of ESC

That's the reason: by default, Tab removes items from slots.
