EDIT: Forgot two things.
Well that might explain it. You didn't stick around long.
Don't question what I am also about to say next but..
I litterly sat in the security room near the medical lab so I could trap it in one of the rooms. Only when the medical lab trap was a success, I waited a while longer and it suddenly spawned out of the lab. My Guess is it moves through doors like a ghost when not active.
OOhhh. I also did wait in the security room until I got the monster trapped in the med room. I also prayed that the monster wouldn't magically appear somewhere on my way to the elevator.
I walked outside for a bit with no problems, it was only when I ran on that tunnel area with the collaped corridors that he spawned near me, guess it's defaulted to do that but it just sucks after trying to trap him for so long.
Sometimes you can hear crawling noises from the ventilation system near the ceiling. I guess the monster uses the vents to move around. Well, it probably just respawns in reality, but I think that's how it can be explained in the game. Perhaps the noises can serve as a warning that the monster is gonna teleport.
I suspect this is a case of cheap tactics to keep the game scary/challenging, rather than official teleporting.
I've locked the guy in the med room and I just run for the elevator, he haven't spawned and I though it probably frictional have done it right.
But no...
As soon as I've started to mess around with the locks, here it comes full speed and hit me :D
