What I would like to see is ladders being something you can interact with with the F key (or whatever key you have bound this function to)
Go up to a ladder, press F to grab onto it, then up takes you up and down takes you down. Then press F again to get off it.
This would remove the problem of thinking that you are getting on the ladder but just falling down instead.
Also, yeah, I definitely am leery whenever I need to go down a ladder. Fall damage in this game is no joke - and if you move too quickly the game will make you fly off the edge as opposed to go down the ladder.
Either it needs to be much smarter about this, or as bkeniry above said, change it into an "activate to grab/get off" deal. Only problem with doing it that way is that you then lose the ability to step off to the side mid-height, unless that's kept.
