No, Richard is not the 'Grim Reaper', and no, there is no Limbo going on. Every character in the game dies by the end, like it or not. Richard is Jacket's insanity, whom scolds and reminds those he visits that what they do will serve no cause in the end. Richard appears to these people as a subconscious persona as listed: The Fans, The Henchman, The Son, Richter, Evan Wright, and Martin Brown. Richard is the one who foreshadows that everyone is heading down the wrong path, and subliminally points out to those whom he visits that they are all going to meet a sorrowful/deadly fate, shall they keep doing what they are doing to put themselves down a much more dark path. Richard wants no remorse or redemption from anyone he vists subconsciously, what he wants is to observe and see the inevitable end of those whom he has warned be either abandoned, or killed.
