CUSTOM PORTRAIT SOLUTION
After some effort I have successfully managed to insert my own custom portrait for the protagonist. Here's how:

1. Get the Unity Asset Bundle Extractor tool (UABE)

https://7daystodie.com/forums/showthread.php?22675-Unity-Assets-Bundle-Extractor

2. Open resources.assets in WIN/TidesOfNumenera_Data folder inside your Steam TTON game folder

3. Find textures Portrait-LCO_Male and Portrait-LCO_Male and extract them as tga using the Plugins button on the right

4. Edit the textures in your preferred graphic editor

5. Import the new textures to replace the old one using the "edit" Plugin in UABE

6. Save the changes resources.assets (this will create a new file sharedassets4.assets)

7. Replace the original sharedassets4.assets file (located in the same folder as resources.assets) with your new sharedassets4.assets

8. Play the Game with your new portrait

EXAMPLE HERE:

http://steamcommunity.com/sharedfiles/filedetails/?id=617687494&fileuploadsuccess=1

CAVEATS:

A. You may have to start a new game for the change to take effect (there is a separate SharedAssets bundle called "player" with another instance of the player portraits, which I haven't managed to edit with existing tools).

B. Step 5 may require some fiddling, in particular using the Raw export/import function first to get everything working. You may have to experiment with exporting and importing existing portraits before going for a fully custom portrait.

MAXIMIZE IT:

By using the same method you can edit basically any texture in the game, including companion portraits, armor etc.

I add this post from:

for some additional info/help to change the portraits in the game

1. Get the Unity Asset Bundle Extractor tool (UABE)

https://7daystodie.com/forums/showthread.php?22675-Unity-Assets-Bundle-Extractor

1. Open resources.assets in WIN/TidesOfNumenera_Data folder inside your Steam TTON game folder.

2. Under view -> search by name -> search Portrait-LCO_Female or Portrait-LCO_Male. Then click on plugins on the right hand side -> extract as .tga, save somewhere

3. Open the file in your preferred graphic editor, edit it or replace with new portrait

4. Then go back to UABE, plugins -> edit -> texture: load (choose your new portrait file) -> ok)

5. Under "view" click "continue search". There are two portrait files that need to be replaced. Both are labelled Portrait-LCO_Female or Portrait-LCO_Male, both will be 2d texture files. Repeat steps 2-5

6. Save the file as resources.assets somewhere (such as the desktop)

7. Replace the original resources.assets file after backing it up

8. Play the Game with your new portrait

Edit: Also this should work for companions too, they also have two 2d texture files that need to be replaced. It's Portrait-CompanionName, so Portrait-Aligern, Portrait-Callistege etc.

I found this wallpaper which could be used for companion portraits / female MC; http://imgur.com/a/E2BJ4

Even if just used for the female MC it doesn't look TOO out of place in game; http://imgur.com/a/r5mqQ

Or SkyCid's awesome one; https://www.reddit.com/r/Torment/comments/5wz03x/custom_female_portrait/

I was looking for the portrait of Rhin in resources.assets file, but i could not find it. Maybe someone can help me out.
