if this is the case, it would kind of explain everything, seeing as the way the faster refresh on tvs work is to kind of make up an extra scene from nowhere (very VERY basic description its odd and complicated and i dont fully understand it lol as they are made to make something form nothing essentially), which would not actually result in 120fps visually but merely the tvs base refresh rate as thats all the data the game is able to show regarding the fps as the extra frames are meade up on the tvs end.
or it oculd be something completely different.
also just noticed you said hdmi ... hdmi can only display upto 60hz.
no tv's actually are capable of recieving a 120hz signel not "just a few" the tv's just upscale it to 120hz by merging frames which is why they all have signal to screen lag.
oh and hdmi v2 can display 1920p resolutions at 120fps. although it is a new standard and i'd be suprised if he had a tv that has hdmi 2. the new gtx 9** series uses it, although i've moved over to display port now for my rog swift

Its a VISIO M321i-A2.
This only started happening when i updated from the previous drivers to the newest. NIVIDIA Control Panel and my TV system info both say im at 120Hz at the desktop. idk about when im in game.
