A mix between medium and high with water on ultra, I will turn other settings down if I have to for that beautiful water, lol. I'll be happy with 40-60 fps, I supposedly meet recommended requirements so I hope I'll do fine.

Assuming not much has changed from GTAIV can play in Ultra, no problem.
1.) The E6600 is only a dual core processor; if it were a dual core with hyperthreading (I-3 and higher) you might have a chance, but i dont think an E6600 will cut it.
2.) GTA IV released 7 years ago...
I have an i5 3470 with a GTX 750 TI and my GTA IV has frame drops where it goes from 90 to 30 fps, so it's either the bad optimization or I have a terrible virus lol.
