GTA V Grahpical Stretching Error

Is anyone else having random textures be stretched accross your screen like this? It happens a lot and is quite annoying during gameplay.

Reminds me of the Call of Duty 4 Shipment map where you'd jump while a frag grenade exploded underneath you and your dead body texture covered a large portion of the map by "stretching".

Also on a side note: anyone else crashing with the "[ERR_GFX_D3D_INIT] – Failed Initialization. Please reboot and restart the game. Try reinstalling the game if this persists." error like I am?

I got this too, then it went away after alt+tabbing.

I noticed on online gameplay, after using the steam menu, during the gameplay. But was some dark texture stretched across the window.

Didn't even see how bad my mirror looked. I just made my texture settings from Very high to high to smooth performance from 30fps to around 50-60 fps. I was talking about what was directly in front of me while driving.

I'm back and forth between alt+tabbing the game already, so I'm not sure if that'll do the fix.
