First of all, if you do use ingame vsync, make SURE you don't have it set to half, as this will keep it at half your screen refresh rate. Most people play on a 60 hz monitor so this would keep your game locked at 30fps. I don't believe this is your issue however...
If you start the game with vsync, anytime you drop below 60 fps ingame, the game will actually take it down and lock it at 30 instead of hitting frames between 30-60. This is quite obviously annoying as all hell and can induce a very lurching/lagging feeling. This is due to the game having a broken buffer when vsync is enabled.
ALL YOU HAVE TO DO TO FIX THIS is everytime you start the game, once your character is loaded up into the world, simply hit ALT TAB back to the desktop, then back into the game again. Now you will still have vsync on, and no screen tearing. And instead of locking to 30fps everytime you drop frames, it will hit the appropriate frames, and never lock you to 30 again.
Now, if ALT TAB isn't an option for you, simply change a graphical setting (like shadows) up or down a notch, and this will also fix the vsync buffering for this play session.
Keep in mind you have to either alt tab or change a graphic option EVERYTIME you load the game up in order to have a working vsync with working buffering.
