I'm not so sure that a 32bit patch for 4gb is going to fix gtav - a 64bit program.
(and I'd definitely verify cache to undo a change to the gta5.exe... like.. seriously)
I'd definitely look at all of the previous listed things first since you've left no other details.
As far as gtav being on D - I personally run mine on M and D - the drive means nothing to steam.
