Report a translation problem
in full-screen mode, while your controller is plugged in
hit alt+enter on your keyboard to minimize the game, hit alt+enter again straight away to remaximize. Do this 2-3 times, then try your controller again.
This fixes it for me when it happens.
Thanks for the tip. But that did NOT work for me. I even tried spamming alt+enter and its still bugged.

Heh, that would make me sick. I've never had this with a wired 360 pad so I'm thinking....does this pad use Direct Input or XInput?
You could maybe look at x360ce.[www.x360ce.com] The only issue there is you need to place .dll files in the game directory and I have no idea how Rockstar are checking for cheats right now but it should be fine for single player.
Someone who uses it might be able to chime in and say if it's safe for online but it would be an interesting test to see if it solves the problem.
