When you kill people and your mental state goes up, the game warns you that higher mental states will make you more of a target.. it makes your white dot more red.. a light pink dot is someone that's maybe killed a couple of people.. a full on red dot means they're on a rampage..
I'm not 100% on this, but I remember a couple of years back, I made a joke in a public lobby asking: "If I take a hit from the bong, will my mental state go down?" and people answered back yes.. maybe they were joking too, maybe it works.. but I've never been mowed down simply because my mental state. People don't care if you're killing NPCs so much, it's when they see so-and-so killed somebody over and over again that people end up going after so-and-so..
(The ONLY time I ever use caplocks is if I'm riding a bike.. which is pretty much only the First Dose 4th mission)
Mental State has always been an easy trigger for "grinders". If they see this stat high on your character they are more likely to kick you from modes you join or remain passive mode around you, since they think you're hostile, and that your entire playtime involves killing other players, something that also increases the mental state stat. You might just like killing npc's but it's seen as a warning sign for anyone who is... shall we say, a little frightened of confrontation.
