Were the female player characters designed to be lesbians?

And judging by the female player character, she is athletic, strong, muscular, bodywise, so why can't I knock out a female bodybuilder in 1 punch just like I did with the hookers?

Yes. First one is XB360 and second one is PS4 and I've also done it myself with a female character on PS3 and on PC. Literally just create a female character in your second slot and then go pick up a hooker.

My char is female but I guess I just never bothered to look around for pros titutes in the online game before. Now that I know they exist I actually saw a couple right away... crazy. o_0
