Cant launch Avisa?
It says press W to exit Kosatka but nothing happens? I tried this while submerged and also while above water.

what you are having issues leaving the Kosatka? you do hit W, if this doesnt work, restart the game. it may be a glitch. I had the same issue with the sparrow the other day, would freeze my screen in game every time I tried to exit. could change lobbies, but same thing. restarted the game to fix it

I know you can buy it in the store and it equips automatically when you exit into the water. Not sure about the scuba gear on the sub though.

pretty sure in the kosatka next to the sub is a icon for scuba gear you gotta equip it from there im pretty sure, unless you just go ahead and buy it from I believe ammu nation
