instant death in water (bug)

i die every time while i go into the water (ocean or any house pool). my character just cant switch to swimming. he will die immediatly if he goes into the water. also if i play in mission and die in water, i cant return or skip the mission. i have quit the game everytime and restart the game again if i accidently drop in the water. this makes me insane, and i have a very poor game experience. i cant find any resolve from online.
and more, sometimes when i am driving the car around the city, people on the street are dying for no reason. then if i get out the car, i also die as soon as possible. how can i deal this problem!!!!!!!!!!!!!plz help.
plus*: i dont play online. i only play story mode... so there wouldnt happen if anyone cheat.

Have you tried verifying the game files?
Have you used any mods?

i do downloading two mods in my game file from https://www.gta5-mods.com/. one is Script Hook V, and one is Mai Shiranui Dead Or Alive 5 [Add-On Ped) (this ped is not useful, cus i cant do mission if i changed my character). However, after i delete all the mods that i have. and keep game back to original. the instant death is still happening in my game...

Are you sure you play GTA 5 and not Vice City?

fixed. it might be mods problem. i re-install the entire game and removed all mod files in my pc.

Sorry. i tried to test removing the mods before i posting. however, i didnt try re-install my game. also, i only download two peds and related mods from https://www.gta5-mods.com/, and both are highest downloading rate on web. Therefore, i thought it wounldnt been the problem of mods.
but i am wrong.
