You'll need to do it manually, there isn't a program that does it for you. You will also want to download the GTA V scripthook (And scripthook.net, unless they was the same thing now, I think they was different back with GTA 4) which will let bigger more script heavy mods be used. Note though, if you have Scripthook installed, and try to go online, scripthook will cause GTA to crash. This is to help stop people who have installed mods from going online and getting banned for the mods.
It keeps all mods in a seperate folder, when you launch single player through the manager it copies your mod files to game directory. When you stop playing it disables them again, and the "Nanny mode" cleans your game folder of any modded files or folders.. even the ones not installed with the mod manager.
When you launch Online play through the manager, it doesn't enable any mods and you can play online without worrying about getting banned.
