i would suggest:
(kind of obvious on this one) steering and looking around put on the joysticks.
accelerator right trigger, decelerator left trigger.
shift up right bumper shift down left bumper
toggle engine break DPAD up (make sure its toggle engine break so it stays on without having to hold the button)
DPAD left and right turn indicators
DPAD down cycle wipers
X activate, that will get you the trailer hook up and disconnect, rest stop, fuel up, and interact with any of the stop here and push button things.
A engine start stop but also have the game set so it starts the engine when you press the accelerator.
use Y for set cruise and B to increase then if you want to slow down just turn off cruise and hit the engine break until at the desired speed and turn cruise back on
leave everything else on keyboard
just my passing thought on how i would try and set up if i was using an xbox controller.
(after thought: if the joysticks can be depressed to act as 2 more buttons you could use one of them to toggle truck lift axles and the other for trailer lift axles, or bind both to one and use the now free button to toggle beacons and or hazzards)
If you mean manual like actual manual shifting, you'd need a secondary controller like an actual shfit stick, or maybe an external analog device that supports the shifter layouts in the game.
Triggers - accel and brake
Bumpers - left and right indicators
Y - flash lights (game calls it 'light horn' if I recall correclty)
X - trailer brake
B - activate cruise control
A - horn (because beeping at people is important
Dpad up and down - increase/decrease cruise control value
Dpad left - change zoom level of dashboard GPS
Dpad right - resume cruise control at current setting
Back and start buttons - decrease/increase retarder
Left stick - steer
Right stick - look around with the camera
Left stick click - zoom in view
Right stick click - reset camera to forward internal view
And if anyone is wondering where my gear keys are for the real auto: I have a Logitech G13 Game Board beside me, and I use the thumb stick on that as a mini sequential gear lever when taking off or reversing. Flick it upwards to put the truck in drive, downwards for reverse.
LT and RT: Brake and accelerate
LB and RB: Turn signals
Left stick: Steering
Left stick click: Parking brake
Right stick: Look around
Right stick click: Toggle high beams
A: Activate

I don’t care about having a horn button since it doesn’t do anything, so usually the only keyboard key I need to press is O for beacons when I’m pulling an oversize load.
That said, when I use my PS4 gamepad, instead of my Wheel/Pedals setup, I have all the basic, most used stuff that would be in easy reach of the steering wheel on the Gamepad, and everything else custom mapped to my Keyboard.
so, for example, on an xbox pad, ABXY would be Cruise control On/Set, Increase speed, Decrease Speed, and Resume, then my Dpad buttons on the left are Up=next Camera, Dn=Interior camera. left=interior look left, Rt= interior look Right. Left Joystick is Steering, right joystick is manual camera control, right trigger= Throttle, left trigger= Brake, right bumper= right turn signal, left bumper= left turn signal. other PS4 buttons I have mapped to ESC or TAB or Map, etc. All my "dashboard" type controls I leave on my Keyboard; Lights, wipers, hazard flashers, Beacon, air suspension; All on the keyboard.
For Shifting, I use Realistic Automatic while using the Gamepad, and the default Keyboard keys {shift} and {CTRL}
I'm on KB/M as well. I've thought about getting a proper wheel-and-pedals setup, but I have a cat with a nasty habit of breaking things. I'd have to secure a wheel so tightly to the desk that there'd be no room to use my computer as a computer.
button assignment using Steam overlay. Open steam look for game and then find controller icon... choices are almost limitless . Button assignment can be double duty. Analogue sticks can be tuned. I use Manual shift with a single press neutral command to get down low fast.

Ive thought about getting a wheel with shift paddles and a button box.
I just cant justify the expense to myself. i have a wife, 2y/o, one OTW, and german shepherd that rely on my income so there is a long list of things that take priority to apparatus to use for at most 4 games i have 3 of which i havnt played in over 2 years.
maybe in a couple years i'll suggest it to my wife for a christmas+birthday gift combo thing lol.
