Hi grox,

Go to multiplayer or coop and press twice the each participating players "up" button. By default, 1st player presses "up arrow" and second player presses "w". The keys are shown in the screen and can be changed from "Keyboard bindings" in the options menu.

When 2 or more players are shown to be ready, an arrow appears in the menu and by pressing enter or the up keys of the participating players and you can continue to level selection...
