Secondly, I found a fix. On the off chance that anyone runs into this in the future, and they themselves haven't found another alternative, one method that has worked consistently for me over five different mod lists some with Musashi's fixes and some without, was to unequip the afflicted soldier having the issue.
I use a squad manager, so for me I just click "Unequip all selected soldiers" and restore them to stock. I know vanilla has a way to make weapons/gear available and it basically does the same thing.
For me, the cause of the issue is when Reskinning weapons. Now it's not every time, but on the off chance I reskin a weapon, equip it, and reskin another, then equip that, whether by accident or neglect, I basically stack 2 weapons on top of eachother, with no way to get back to the bottom where the FIRST weapon was equipped.
