Yup. Found that out the hard way, myself. It's partially realistic, patially there to give greater weight to the game's "attrition" systems. You have to wait for the soldier to heal, then wait some more for them to be treated for mental issues.
You prob have a mod that stops it from happening if you can not treat your personal with full health.
My templar was my best soldier and I kept having to send him into missions while tired, kept getting negative traits over and over again.
fear of the chosen is generally something that comes from chosen sabotage, if you kill the chosen that did the sabotage (attacking its base and killing it in its coffin) the effects of the chosen, like their sabotage and dark events end immediately.
using the medical facility removes them more rapidly but if you just wait a few months after having the facility on hand the traits seem to dissappear on their own.
