Apparently there is also a bug where you can lose your head but still be alive and play normally.
Hopefully Tango will get busy on patching up bugs and optimize the game a bit. I just hope Bethesda won't do like they did with Skyrim cutting support leaving issues unfixed or the community to fix what issues they could. The silence is concerning, I would've thought they would have announced they are working on another patch. No public beta support to beta test patches before they go live, at least Capcom did that much with RE4:UHD.
I encountered this bug twice during my playthrough and seems to happen randomly when going down a ladder or when ending up at the hospital either through the portal or after a cutscene.
It's a trivial bug that happens randomly and just a minor annoyance when it happens, be sure to save frequently until the devs patch it.
