There is no health bar. The screen turns red when you take damage, until you take enough and then die. You're probably looking at the dead eye meter.
Yeah, you're likely mistaking the Dead Eye meter for health. This game uses a regenerating health system with no visible health bar. The red bar is Dead Eye, the blue bar is horse stamina. The horse doesn't lose stamina unless you're tapping the sprint button for a full-speed sprint. When the stamina bar is a lighter blue, it indicates you're on a road or a path, allowing your horse to run slightly faster. If the bar turns a darker shade of blue, you're riding in an area where the horse can't run as fast as on clear paths.

classic rockstar games putting the running button for left shift, press it more than 5 times and boom suddenly it wants to enable sticky keys, instead of holding shift i have to repeadetly press it, how suprising, as for health bar, why did they not allow one, if they sell you medicine, which you don't know when to use if you can't see your health bar.

Just rebind the key then? Change run to hold in settings?

I definitely suggest going to Options > Keyboard and toggling "Hold [LShift] to sprint (On Vehicle)" on. After that, holding LShift + W will have you sprinting without the need to tap. Let go of W and just hold LShift to continue galloping at a speed where your stamina regenerates.

About the medicine, it can actually come in handy (especially on Hardcore), you should eventually get a "feel" for when you're about to die, but usually you're at death's door when the audio starts to get all muffled. There's a pretty big buffer on Normal before you actually go down, though sometimes they can still basically instakill you out of nowhere.

You know because the screen turns red and the audio gets muffled. It conveys the exact same information as a health bar would, except you don't know the exact amount of HP. Which is irrelevant anyways because medicine restores your health fully no matter how much exact HP you got. It makes the game more immersive and also reduces HUD bloat.

Also as an aside, unless you are playing on hardcore mode you frankly won't ever need medicine anyway. Health regenerates fast enough while in cover. You won't need most consumable items on normal mode in general.
