Playstyle IS the difficulty setting. There's also nightmare mode as well.
Yes it is. Select "Nightmare mode" then select "Single player" from the playstyle list.
you can also choose to change to single player in the options menu too.
The game just doesn't use classic "difficulty levels" beyond that.
Edit - Besides, it's not really something you actually need to worry about. In 50+ hours of playtime, I had a grand total of 3 or 4 people randomly join my game. The rest of the time I was on my own.
(before someone decides to be a smart aleck and say that's proof no one's playing the game, I tend to play early in the day or late at night. I wouldnt' expect to run into others during those times.)
