Episode 3 Add to the Collection: The blue icons for the mission dissapeared because all of the captians were killed before that mission was complete and it went into a cutscene. You can still bring back the bikes and they will still count and the achievement should still unlock. The bikes must be parked inside the store and Torque will make a comment letting you know you successfully delived the bike. All bikes must be returned before you face the last boss. If you blew up a bike or got it stuck then you will have to restart. Youtube the locations if needed.
Episode 4 Sworn to Protect: The suvivors are avialable right from the start to rescue. There are 20 to save, 5 in each district. Just start a new playthrough after you finish the current one you are on and rescue them as the first thing you do. Youtube the locations if needed.
Episode 4 Ashes to Ashes and Bootleg Operator: *YOU HAVE TO COLLECT ALL IN ONE SINGLE PLAY SESSION* Some do not appear on the map but are still able to be collected. Youtube locations and you may have to revisit all of them to find the ones you missed. There is another glitch as well which is the one I currently have which was caused because I did not collect them all in one session. This glitch makes it so when you collect a biohazard it does not add to the count, so for example you have 37 then collect another and it still says 37, instead of 38. The only way to fix this is to remove your save file and start an entire new save file (which means you will be level 1 again) and then replay the DLC collecting all in one single session. I have not wanted to make a new save so I have not done this yet.
anyways, thanks so much for the help! i was really confused about some of the Achievements for the Untold Stories. TBH, im not surprised that Capcom would be lazy enough to leave bugs that prevent you from getting Achievements, considering that Dead Rising 2 and Off the Record have a few extra bugs in them AFTER the Steamworks transition (such as crashing when leaving PC Settings Menu and Chuck and Frank's in-game movements being interrupted every 10 seconds) that werent even in the Games for Windows Live version to begin with
