I've made dozens of trips between my cove tree base and safe shallows base without ever encountering a ghost leviathan. Maybe it's just on the far edge of the chamber out of visual range?
As far as I can tell the map I’ve linked bellow is a fairly accurate map of the LR. It shows where the ghost leviathans patrol the LR with blue circles.

Ghost leviathan? I'm not bothering him so why should he be bothering me?
I make sure to always keep one or two beacons with me at all times so I can leave them as soon as i find something worth coming back to.
I've got at least two beacons marking the entrances to the Lost River.
Feels a little redundant given I also play with the map mod but it's something I can look at while piloting the cyclops.
But there is a ghost leviathan in the bonefields section! Thing is he seems to usually hover closer to the path that leads to bonefields from the deep grand reef then the path leading from bonefields to the B.K. Trench. As a result if you take the B.K. trench route and hug the wall you usually don’t get close enough to see him. But there is one nearby!
Yes, good point, as long as you stay along the left side, you never see it. If you go along the right side, you would run into it.
