I've just finished the game with doing everything. Got all the other achievements except the last one for 100% completion. Now, the strange thing is that the map shows only 99%. Is there a secret somewhere or is this a bug?

It seems that I'm unable to get achievements to register, despite fulfilling the conditions for some of them. Is it possible for this to be looked into?
