Blank Game Icon
The icon of the game is just a while blank file.

Any fix on how to return it to the original star icon?

This is simple and will work for any game, you can even change an icon if you don't like it!

1. On the desktop, icon right click (or left, whatever brings up the white box for you) and click properties.

2. Go to the "Web Document" tab and click the box that says "Change Icon..."

3. A "Change Icon" window should pop up. On it, click "Browse" in the top right corner and go to "C:\Program Files (x86)\Steam\steamapps\common\Call of Duty" You should see the .exe files for the original Call of Duty and United Offensive's single player and multiplayer, each with the star icon. If not, make sure that the dropdown box in the bottom right says "Icon files" in it.

4. For United Offensive, click either CoDUOSP or CoDUOMP, the ones with the orangy icons. They are both the same icon so it doesn't matter which one.

5. Click "Open" in the bottom right and you should see the icon in the "Change Icon" window from earlier. Make sure the icon is highlighted in blue, click "OK", and the window should close. On the original Properties window, click "Apply" in the bottom right corner and then click "OK".

All done! you should now have the proper icon. If you want to fix the original Call of Duty's icon, simply follow these same steps, just use CoDSP or CoDMP, the ones with the black icon instead of using the orange United Offensive ones. In the future, if there is any game without an icon, just do this with that game's .exe in its files.

Edit: One more question, why do some icons that are actually large appear small sometimes, and then grow back to their large size, and then shrink to their small size again?

I really don't know, I would guess that some icons are lower resolution, so sometimes they appear as the original resolution size, then they grow and get stretched out into icon size(?) I don't know a way to fix it. Maybe copy the icon image and find a way to upscale the copy to a higher resolution? It may not be anything to do with the resolution, I'm not an expert, that's just my guess.

Also, I tried to use custom icons from the internet but it doesn't work because the pictures are not in an icon format or something. Any way to fix this?
Try saving a picture as a .bmp, or rename it as (the name).bmp. Then when you're picking the icon for the program, make sure the dropdown box in the bottom corner says "all files" so it shows the .bmp. However, it wont be transparent, just a square, so any empty space in the icon will be white.
Correction, sometimes they do turn out transparent.
