edit: got it somehow on the 11th attempt by cutting of the body again in the final location. don't think anything was different but idk.
Getting this costume requires to complete the mission with 25 (or maybe 26-28) casualties. Guides say you need to cut off body but it's a lie. I got it without cutting off and without Betty (I used Blake as second Rarity). Also I saw an opinion that it requires to complete the misson without dying, thats also not true. I'm actually not sure about maximum amount of casualties required, but I didn't get it with 29 and got with 25 two times (I beat the game two times). Check your casualties amount in Bind Break it's most likely at least less then 29 now.
Also casualties amount randomised in this mission, I noticed when starting from checkpoints it raises by some amount but it differs randomly, so I recommend to reload few times after each time casualties amount raises and continue through the mission when it's low.
yes, it had to be the bodycount. mine was at 25 in the final run and I unlocked the costume. it was so confusing because all other items before unlocked automatically trough shrines or by cutting off that body from the boss. anyway, case closed :)
