Denied healing
I got porphyria and went to an altar to get a blessing but I got a got a message about being a wretch and repent. I'm not sure where this is coming from but maybe because it happened after joining the thieves which gave me 2 infamy. But I still had a bit more fame than infamy. Can't you get blessings if you have any infamy?

I think you can't get blessings as long as your infamy is higher than your fame.
You will now need to make the trek around the map activating all the wayshrines. It's one thing I don't like about Oblivion. You become the Grey Fox and wear the mask and it's instant blacklist from using the altars, for example. But you can Dark Brotherhood your way around the map with relative impunity, lol.
Yup, either wayshrines or cure disease potions/spell is what you'll need

Closing an Oblivion Gate should reward you 1 point of fame. Drink some invisibility potions and book it to the sigil stone.
make sure to get it cured, drink a cure disease potion if you have to, as curing vampirism is a big pain in the ass compared to Skyrim if you let it progress to becoming a vampire

Go to Bravil and search forest around Anutwyll ruins for some mandrake root and eat it.
Yeas but the thing is I had I think 4 fame at that point, more fame than infamy. Makes me wonder if you need to have a substantial ratio of fame/infamy to avoid trouble?

I think it's denying you the healing because of porphyria. I might be wrong, but if your fame is a little higher than your infamy then the altar should be letting you heal. What you need is a healing potion, unless you want to be a vampire.

By the way I get blessings with 11 points of infamy just fine, but my fame is about 40-ish.

This. But to add an explanation for those unaware: NEVER make Cure Disease potions to cure disease with. Just eating a single ingredient with the Cure Disease effect cures diseases as there is no magnitude associated with it. Eating one ingredient works as well as drinking a potion made of two.

BTW you also can't get healing if you have a bounty. Pay off your bounty if you have one.

Eat the pie in the warrior guild of Anvil, it cure diseases.

Not really you can just use the purgeblood salt altar
