It depends on how good your stealth is to begin with - so you'll have to sneak around the city for no reason, in order to level your sneaking ability. But in relation to your question, are you wearing any heavy gear by chance?
My Sneak level is above 50, the difficulty is set on Adept (medium) but once i hit the enemy then it always knows where i am
i thought that was affecting it somehow :/ but yea as i said before, my sneak level is above 50, it must do something right?
if it's like you said then i probably mixed it up with how i played skyrim, but man it really feels bugged the stealth...
Yeah that's weird man. I literally JUST now killed an NPC while another was standing right next to him and the other did not notice me so I stealth killed him as well. I am not even level 50 yet in sneaking. It is strange that your results are so wildly different from mine.
Wait, if i one shot them then i have no problem even if someone is standing next to them, the problem is if they don't die at the first hit then everything goes to ♥♥♥♥ permanently.
Like you, I can 'snipe' someone across a long room and they instantly "full-eye" and charge me, that's with 75+ Sneaking. Same as you, stuck in 'combat' unless I run away for 5 minutes. Even if I pop invisibility (blink) and map transition etc, they still continue to track me.
That alongside my bows getting "Sneak Damage x1" or not applying at all - then sometimes x3 is making my build a little bit janky to play right now but I'm working through it xD.

I am not being sarcastic, just to get that out of the way first. What is the expected behavior after the first hit? That they don't notice you? It seems normal to me that after being injured they'd up their game and notice there is an intruder. But then I haven't played the game in over 15 years so perhaps I just don't recall what is supposed to happen.
From what i remember in the original game after the enemy gets hit it goes in "alert" mode and they come inspect the place where you shot, but without having the player seen, so if you move after the first hit and you're still in "sneak" mode, you can manage to evade the enemy.
In this case instead as soon as the arrow lands on the enemy, the enemy exactly knows where you are and even if you move away, behind a wall or whatever the enemy can still see you and it will chase you forever...
