The sewers were fine but the frame rate went to ♥♥♥♥ once I hit the open world. I can't even get 60 FPS on medium settings.
I've seen players with much better specs than me also say they get low frame rates so it's looking like it's just bad optimization.

Why edit your first reply just to type "upgrade potato?" And from looking at your profile, we're playing similar "potato games." Only you got waaaaaaaay more free time on your hands obviously.
Yeah, this is one of the main problems. Feels like a memory leak. I think the game keeps loading stuff into memory, but doesn't remove unneeded data properly. So it just gets full after a while and chokes.
It runs better for me in caves and interiors. Soon as I step outside the FPS drops immediately. Save and reload will start me off at 30 FPS but then it quickly drops to single digits even on low settings.
Many Gamepass reviews say this game ran fine before some recent patch update that removes DLSS and frame generation options. It's definitely something on Bethesda / Virtouso's end.
I have everything at Ultra, DLSS on Quality w/ FG on, get around 100-120 FPS in Open World, 170 in indoor areas. Have had a few crashes, once on loading and most others on closing the game.
Do have stutters tho. Usually in Open World, not so much in cities. Honestly think it's more so due to how the original game loaded cells as you moved through the open world.
I've also heard a few people say that FSR seemed to have less stutters than DLSS, possibly a bad implementation of it?
Have you gone through the different settings and trying to set each one to low?
There might be one or two that are tanking your system.
Hardware lumen will do that.
Try going software - low. Game still looks amazing with it.
If that doesn't work, try the Lumen begone trick with the engine.ini file.
Shadows on vegetation and indoors will look like dark noise, but the game should run way better.
