I have a potion I make that gives Chameleon for a long time, something like 2-3 minutes long. It seems that if you load a different area, or repeatedly apply the potion Chameleon you'll be locked in this Chameleon state, without the benefit - just the visuals.
1 Solution is to use a Chameleon Spell, or a Chameleon Potion you find in a chest (not made), and let it expire. This one has been the most inconsistent for me but it does work.
The more consistent one is to Quick-Save-Quick-Load. That USUALLY resets it.
Another solution would be to go through the Shivering Isle portal for the first time. And, in a broader sense for this solution; you need to do something in the game that will 'reset' your character, visually. This can be a quest, such as Sanguine's Dreadric Quest, after you cast the spell.
Edit: And sometimes, it just needs to be waited out. I have had something say "Lasts 326 Seconds", which is a lot longer feeling when you want it to go away.
If you start seeing you limbs and head appear, while you have the effect, close the game and restart; you're going to crash soon.
