You say only one hero was afflicted, so that can explain one instance, but unless you had Imposter Syndrome (Negative Quirk: 4% chance to pass per round), Crimson Curse, recovering from stuns, or other specific issues, then it's stupendously unlikely for any one hero -- nevermind all of them -- to pass their turn over and over, so there is clearly more at play here.
Speaking as someone who deletes saves the moment anyone dies, it's worth repeating that doing so (outside of Stygian/Bloodmoon) simply isn't worth it outside of personal preference. Heroes are a dime-a-dozen, and resources/resolve is easy to reacquire, so take it as a lesson about the importance of avoiding 100 stress and practice it while rebuilding a new squad.
Recently I had a squad of 4 spiders take the first turn followed by the second round before any of characters could act and all 8 attacks were on the same person. Needless to say that character didn't make it home.
I mean , maybe 1 more was afflicted but DEFINITELY no more than two. thank You for Your input, I took it to heart.

Do bear in mind that it's entirely possible to complete deathless runs consistently in this game, however unlikely it may be for new or blind players. Death tends to creep around the corner unexpectedly for as long as people don't know how to keep it at bay fully. That's not an insult, either, but a testament to how boring the game can get when you know how it works (compared to when heroes felt like they could be snatched at any moment while learning DD).
Above is one such example. This is an issue people will commonly complain about when lacking a very specific stat. Spiders just happen to be one of the best at abusing it (due to mark-stun-spin setup combos), but it applies to many enemies. So long as you are equal or slower, enemies of many kinds will consistently put players on the backfoot during fights.
Just remember -- what the enemy can do to you, you can do right back to the enemy. If the AI was a real person, they'd be posting complaints about how their units rarely get to take a swing at a well-greased team.
This circles back to the original post and the mention of affliction. It's surprisingly easy to (a) strangle damage, and by extension (b) reduce or remedy stress intake during a dungeon, thus avoiding afflictions -- which are one of the absolute worst things that can happen during a quest (shortly behind flat-out deaths, because all it takes is one loss of control to screw everyone over big time).
If you get afflicted in the first place, that's a sign something is not going as well as it could, and a good time to consider cutting your losses and taking your current loot home with all your heroes intact. Over time, you'll learn how to better avoid such scenarios, but for now, always keep the retreat button in mind and "Remind yourself that overconfidence is a slow and insidious killer..."
