Leprosy is a real disease, but in real life it definitely wouldn't help one in combat. It may cause a lack of pain, but it causes a lack of other sensations and possibly weakness. Not something you want in a swordfighter.

But he wears the mask for 2 reasons: First, it'd stress out his party. This is shown with the "let the mask down" camping skill. Second, masks are imbued with magic to give him bonuses, as is seen in many of the leper trinkets.
There is this movie (I forget the name) where he is depicted as wearing a mask like the Leper in Darkest Dungeon and everything. Pretty cool. I'm pretty certain the Leper in the game was influenced by that portrayal of the him as they look very much alike.
