Successfully complete first foray into ruins?

I'm on like week 40, I started playing during the early access, and if I go back to week one I can see myself successfully completing the first ruins quest like I know I did. But I don't have the caretaker goal "successfully complete first foray into ruins"...

Is this a bug, and if so, is there a way I can fix it?

Not a bug. The "first foray into the Ruins" refers to a specific Ruins quest which was implemented on release as an extended tutorial. You should have one short Apprentice-level quest in the Ruins with a gold-trimmed icon (as opposed to the usual silver-trimmed icons). Completing that quest will complete the "first foray into the Ruins" goal.
