To acquire the Journal Entries, yes you do. They have to make it home with your party.
I believe they are unique, though I'm not too sure, as I don't really pay attention to the journal entries much.

Just some fluff. But I think, if you got them all, they won't drop anymore - so theres that.
The pages gets added to your collection, that you can check in the Ancestor's Memoirs building in town.

This is annoying and wastes inventory, though, so if you want to fix this, there are mods to autosave journals just like Shieldbreaker's journals autosave.

If you have mods that add journal entries (like Marvin Seo's) you need additional mods to save those.
As a completionist, I'm cool with it.
