You can just not open the courtyard - the bloodsuckers do not spread until you do the first crimson court encounter, but you can get a flagellant any time (Well, any time he appears at the stagecoach). You can get the flagellant that way, but to get any of the set trinkets you'll have to open the courtyard.

Crimson Curse is also not as bad as it seems - it has small hp/resist penalties, but grants bonus speed. It's the 'Wasting' state that sucks, but that's insanely easy to avoid as long as you remember to pack a few blood with you when taking cursed members on runs.

You can also activate the Flagellant without activating Crimson Court for your playthrough. You need to buy Crimson Court to get the Flagellant, but you can activate the class separately from the main DLC.

You can also wait until ~week 25 before activating the Crimson Court. (Hint: You should activate it while you still have a party of L2 heroes to bring to the first CC mission.)

In my playthrough last month, I wasn't originally planning on activating it, but I got off to a good start and by week 20 or so I decided that I wanted to up the difficulty, so I ended up activating it. And it wasn't really as bad as I thought it would be either, but I did read about / look up the missions because they are extremely long (after the first one).

Also if you don't like the crimson curse or some other part, you could just remove it with mods. This game is really modable.
Some mods also add interesting trinkets that use crimson court mechanics.

Nothing at all

It's not that 'nothing at all happens'. It's that the punishment applied for ignoring the court is barely of consequence.

You'll be accosted by a "Town Is Abuzz" event very regularly which overwrites a lot of basic Hamlet events and incurs a stress-penalty to most respite facilities, but it's not like this is a problem if you're decent at managing stress normally. It just means the facilities you might pay for are slightly less effective at the time, and any half-decent player won't care.

However, if all you care about is the Flagellant and have no desire to enjoy the wonderful DLC to its fullest, then you may as well just enable the Flagellant and disable the rest on the menu screen. There's really no point enduring the negative for the sake of it.

Honestly, if you like the core game then there's no reason not to get Crimson Court, and its reputation for difficulty is entirely undeserved. It merely expects you to learn and cope with some new mechanics, just like the core game did as a basis.

You'll get a whole new zone with its own story, aesthetic, mobs, music, bosses and features, not to mention a wealth of trinkets to choose from as a reward. Basically, it adds an extra few layers to the onion without entirely being more of the same-old-same.

thanks guys
Keep in mind that if you do decide to open up the court, expect to make many trips to the court to collect blood. You will not be able to have enough blood to take care of what will eventually become a roster nearly full of cursed heroes if you don't do this. It's not a difficult mission by any means, but it can become tedious since it doesn't directly contribute to progress.

This depends entirely on when you open the court. If you do it the moment it becomes available, yes, you'll have to do some blood runs and probably struggle with a lot of infected heroes at least until you can take on the first boss.

But blood drops as loot in regular dungeons as well, right from the beginning, so as long as you keep it you can easily stockpile enough. I actually really enjoyed the Crimson Court, and I didn't do a single blood run because I waited until I had a bunch of level 2's before I opened the Court. By then I'd accumulated enough blood to have a good start, and I continued to get more as time went by.
