Go to your Graveyard in hamlet, click on your dead heroes, and confirm that they died of "HEART ATTACK", and not something else.

Without details, I'll assume it's reporter error
Singleplayer, no idea what butchers circus is.
I was fighting some little basic pig mob that used some toxic bite or spit in a difficulty 1 dungeon. He didnt hit my people they automatically died when the next turn came around.
All four of them.
All four of my characters had varying levels of sanity. None were maxed out.
One of them was close, but only 3/4 of the bar was full.
Another was like 1/5th.
Other things like stalling the fight can also cause party wide stress.
A hero is at death's door when their HP is zero.
Heart attack always puts your heroes to death's door. If they are already at death's door it kills them. And it always happens at 200 stress.
While on death's door any attack that deals a minimum of 1 damage has a chance to kill a hero. This includes blights and bleeds. And blight or bleed are the only things that can kill a hero "automatically" when they get their turn.
You can check the cause of death at the cemetery in Hamlet.
One way to get absolutely wrecked is to retreat when more than one person is above 100 stress. You can chain down your entire team without seeing the stress bar update, because taking a heart attack causes party-wide stress, which causes a heart attack, which causes party-wide stress, which kills your original guy, which causes party-wide stress....
Otherwise, bug, might be. I know chain heart attacks are a thing if you retreat or have very high stress levels just before someone has a heart attack. Heart attacks increases everyone's stress levels, but a member dying (especially from a heart attack) spikes everyone else's stress. It is possible to retreat from a fight full HP but high stress, and lose everyone in your party to a chain heart attack strike.
