Indeed making it with big damage, focus on killing death doors characters. At the start of each turn set all characters with mortality debuff at death doors (so you can't heal over time). Again, dodge it's important, but protection don't help. The damage could be low but maybe with aoe and debuff.
The idea behind this it's to create a character that potentially can reduce the number of the group, but it's not a threat for the other heros.
Good ideas for a set boss ruins boss but, What I was suggesting was more a roaming one; One that you could encounter at any point during your adventure. Really sorry to say but most of the ideas presented contradict what I was going for.
Not that there bad but, Doing High, Death Door-like damage is a huge part of what makes this boss what they are. And while going for the character on deaths door as if they were marked is a pretty good idea, giving the debuff to all character as part of the battle defeats the point and would make the fight harder than it would need to be.
Though the aoe point is a good one and I've been thinking about for a bit, If they do add this I would love to see a aoe with some very death door synergizing effects. Like Low damage on all but with a (+110) bleed or blight!
