As for the Radiant/Darkest debate I don't think the difference in difficulty between the two is big enough for it to matter. That being said I'd play on Darkest if you think you'll stick around long enough that you'll do a second playthrough and Radiant if you're only planning on playing for a few hours every now and again.
Basically, if you think you're going to lose a lot of heroes and don't like the idea of grinding up new ones, play Radiant. Otherwise play Darkest.
There are no must-have mods although I like having walking speed mods. They won't change balance, they just make you move through hallways faster.
I recently started a game and forgot to turn on Faster Darkest Dungeon and was immediately enraged at how slow the walking animations are if unmodded. Why the hell did they inflict that torture on everyone?
