Losing Items to Refracted Affliction

A couple of nights ago I was doing a CoM run with a Vestal, Thorn(Mod), Crusader and Bounty Hunter. When my Thorn died, she also gained the affliction "Refracted", she remained on my roster. I thought this was a glitch, because the game does not explain what refracted does at all, and she clearly received a death blow. In my irritation, I ended up dismissing her. I didn't know you could just go back into the Farmstead and grab the items from her Refracted form. She had a class specific item on her from another mod, Sunward Isles. Technically, she did die, and so she had no items on her when I looked through her character screen one last time.

Can those items be retrieved from the Shrieker, or is that one class specific, unique item gone for good?

Refracted is the affliction you get when you hit affliction in the Farmstead. There's multiple variations with various effects.

In case you didn't know, when a hero dies in the Endless Harvest they come back a couple weeks later, which is why she was still in your roster.

Trinkets are still lost because your hero did still die. You should be able to get it back from the Shrieker, yes.
