The crimson curse phases last for a certain amount of time, there is a minimum and maximum duration for each phase, the actual duration is determined randomly each time. Presumable the trinket makes the phases last longer, effectively reducing your consumption of the blood.

The phases last some 30 rounds or so, but the ancestor's vintage will prolong said phases, by how much I don't know, because I never tried using it.

so the way the Crimson Curse works is that it gets progressively worse across three stages (Passive, Craving, and Wasting) until the infected hero eventually dies. Giving a hero The Blood will reset this progression and stop the hero from dying. The Ancestor's Vintage will make it so that each stage of the Curse lasts longer so that you don't have to use the Blood as much.

Do you instantly die from the curse, or is there a deaths door check?

There's 4 stages: Wasted (wasting), Craving, Passive, and Bloodlust. Using a vial of blood will advance you 2 stages, so from Wasting --> Passive or Craving --> Bloodlust (exceptions being when you use blood when passive or in bloodlust.) So using vintage will prolong these stages, or at least the passive and craving stages. Not sure of the specifics.

Haven't let any heroes die of wasting, so can't tell you with 100% certainty, but I'm pretty sure that the hero simply dies of thirst. I remember lots of people making a big stink out of the lack of death's door check.
