Losing Trinkets

I made a few mistakes in my last run and lost some powerful trinkets, Pagan Artifact, Mercurial Salve, and most notably, Dismas's Head. I escaped from the fight and immediately retreated home. However, I've fought the Collector twice and opened secret room chests with a skeleton key since then but I haven't seen Dismas's Head turn up anywhere. Is only able to be earned once and now it's just gone forever? Or am I just getting unlucky drop rates?

Getting unlucky. I got the Vestal's (forgot the name, Junia maybe?) head twice, both times from the Collector.

If I remember right you can get back lost trinkets from the screecher fight or how it was spelled.
Had him unlocked once but never did him, but I guess you can unlock him by losing enough trinkets.

It comes with a special events where he steals your trinkets.

There's two separate Shrieker quests. In one, he's a town event that steals your trinkets, making you have to go get them. In the other, it's an attempt to get back the trinkets he stole from the dead. This second one is the only way to recover lost unique items like a head or an ancestral trinket (except for trophies, which return automatically). The game keeps track of the trinkets you lose, and once there's enough, the Shreiker can spawn for you to win these trinkets back.
