It is meant to max Defender in position 4. But I think personaly it's a bit of a waste to put MAA in position 4. There are not even 4 skills he can use at that position. Well if Bellow could be cast in all positions, that would get interesting. Still useful in some boss fights.

Not really. There is already a much better trinket for that purpose: the rare Recovery Charm providing +40% heal and is not locked to a position. Problem with this defending trinket is you need mostly a main heal because there are not so many off heals that can do the job and the one who could (arbalest) cannot heal at position 4. So that makes MAA/(VES/OCC)/xx/xx. Well maybe the healer could be moved to another place. But this party looks tailored to take on high single target damage like the ones some of the bosses deal.
