Heart attacks penalty permanent?

No, a Heart Attack gives you a more serious version of the death's door recovery debuff, except for reducing stress taken oddly enough, if you survive the heart attack. Otherwise it automaticaly kills the character when they are already on death's door. The debuff for survivors can be removed with 'Mortality Debuff' removal like the Crusader's 'Stand Tall'

Nope, those go away after you return to the hamlet. Quirks and diseases have to be treated at the sanitarium, and afflictions (gotten at 100 stress) are removed when they partake in stress relief.

There is also an Occultist Camp skill that removes the mortality debuff as well.

The mortality debuffs are small but persist unless you use the few camp skill that can remove them or finish the mission. However, if you finish a mission with a hero that had a mortality debuff, you are much more likely to get negative quirks vs positive ones.

3 of my heroes that have heart attack penalties still have them after 2 weeks. Why is this?

Only way to remove these penalties is from camping?

In mission yeah. Otherwise after the mission is over the debuff goes away.

Conversely, there's a few camp skills that have a much better effect if the hero has mortality debuffs.

Sanctuary, for example, prevents you from getting ambushed while camping. But if anybody has mortality debuffs, it gives them a huge heal and stress heal. This is great because often you'll want to camp after one guy got beat up pretty badly, and its likely he's low on HP and high on stress anyway.

Are you sure you're talking about "heart attack penalties" (Mortality debuffs, actually. Nothing by the name of "heart attack penalty" exists)? These will automatically disappear when you leave the dungeon (ANY dungon at ANY time).

Are you sure they're not "Afflictions"? Which you get when you have 100 stress (NOT when you get a heart attack). Afflictions only disappear if you put your heroes in stress relief (abbey and tavern). If you do not do this, they will remain afflicted.

Always think about when the best time to camp. Sometimes you might want to hold off, like if you are doing well. Other times, if you are getting stomped, its still prudent to camp earlier if it might turn things around. Worst case you'll flee with less stress vs never camping at all.

After the 3rd week 3 of my characters that were affected are now all good.

Possible solution maybe restarting the game. For me it got fixed after I closed the game and opened it the next day.

afflictions and mortality debuffs are not a bug, Bruh....
